    Description

    This is an unusual piece in that it is an order form for Opium, Etc. approved by Congress December 17, 1914.  The form is "Series 1915" and says United States Internal Revenue at the bottom.  It is stamped with the name of the purchaser "Dr. Grace Bennett, Brattlesboro, Vt." and is numbered #E329679.  It has not been used and is 9" by 6 & 1/2" in very good condition.
